"Why," you ask? Because we want profile pages to have freedom of customization, but also to have some consistency. This way, when anyone visits a deviant, they know they can always find the art in the top left, and personal info in the top right.

Click on the arrow next to your user name at the top left corner, scroll down and click 'edit settings' then click on 'identity' and at the top it'll show 'change avatar' then you add one. But make sure it is the right size or it won't let you upload it.
